~ PRIORITY ~

Dear Parent/Guardian,

You were asked to provide the school with Emergency Information for your child, including contact and health-related information in the fall of the current school year or when you enrolled your child as a new student within GLSD.  You will be asked to update this information every new school year.

It is imperative that current parent/guardian & emergency contact information and health-related information (including medical conditions, allergies, current medications, recent illnesses, injuries, surgeries & procedures and doctors' names/phone numbers) are on file in the Nurse's Office at school.  The information you provide is treated as confidential. Only those individuals who need to know it for the health, safety & welfare of your child will be provided with it &/or have access to it.  

When your child is in school, it may be necessary for you to receive a phone call because of illness, injury or other reason involving your child.  When you complete the Emergency Information Form you receive from the school, please include parent(s)/guardian(s) home address(s) & phone number(s), the name(s) of your workplace(s), your work phone number(s) as well as your cell phone number(s) to minimize delays in contacting you.  Also, remember to sign and date the form.
  
 
Every effort will be made to contact you before a call is made to one or more of the individuals you list as "Emergency Contacts".  These contacts need to know that they may be called upon to assume temporary care of your son/daughter, in the event of an illness, injury or other reason, at school if you cannot be reached by phone. The individuals you choose as contacts should be family members, neighbors or friends that you know well, who live within a reasonable distance and who can drive to the school to pick your child up for you.
 
If your contact information, your emergency contact information or your child's health information changes during the school year, PLEASE notify the Principal's Office, the Guidance Office or the Nurse's Office either in person, in writing, by phone or via e-mail as soon as possible.

For safety and security reasons, your child will NOT be permitted to leave the school building with anyone other than you or the contacts you have designated as Emergency Contacts.
